I compared this product to regular Preparation H ointment (without lidocaine) as well as Zihyo’s Hemorrhoid Cream with 5% Lidocaine (https://www.amazon.com/dp/B0CTMVPTWS/, currently unavailable on Amazon; was $17.99 after $5-off coupon for 2 oz. tube in April). For quick and effective relief from pain and itching, there is no comparison: both creams containing lidocaine beat regular Prep H hands-down
Lidocaine is a local anesthetic is much faster and more effective at relieving pain and itching than products without lidocaine. (Prep H sells a cream which contains lidocaine (https://www.amazon.com/dp/B07PQY2SLP/ ), but I have never tried it because it is more than twice the cost of other brands.) I would call these lidocaine products creams rather than ointments; they have a light creamy consistency that (unlike regular Prep H) is completely non-greasy, so a quick rinse gets it off your fingers easily.
Comparing this Bottoms Up cream to the Zihyo cream:
– I found the Bottoms Up cream to be slightly less numbing than the Zihyo cream.
– Both creams caused a warming sensation within a few seconds of applying. The warming sensation of the Zihyo cream faded faster (without diminishing the numbing action) while the Bottoms Up’s warming sensation lasted a bit longer than I would have liked.
– I found that the numbing level of the Zihyo cream slightly stronger than the Bottoms Up cream.
– The Bottoms Up cream wins hands-down is in the scent category: it has no noticeable scent while the Zihyo cream smells not-so-great for the first minute or two.
– The prices are comparable: both products are about $20 for 2 oz.
One point of annoyance: I assume these containers are filled by weight, but I was a little surprised/disappointed to open the plastic jar and find it only about 2/3 full (see photo). Ointment shouldn’t settle during shipment, so I was left wondering whether each jar actually contained 2 oz. of product or whether the manufacturer just found a bargain on overly large jars.
As for value, at about $10/oz., the Bottoms Up cream and the Zihyo cream seem like excellent value given how effective they are. Prep H lidocaine cream is currently $26.99 on Amazon for a 1 oz. tube — i.e., *half* as much much product for 2-1/2 times the cost per ounce.
For anyone who has been using greasy Prep H ointment, I would definitely recommend trying a cream product with lidocaine. It is much neater to use and much more effective.
Bottom Line: I found the Zihyo cream to be a little more effective, but the Bottoms Up cream was a close runner-up. If the Zihyo cream continues to be unavailable, I don’t think you can go wrong with Bottoms Up.

I like it I just wish it wasn’t pink. If you touch anything else but where you need it, it gets pick everywhere including your underwear. I also wish it had a better consistency, it’s kinda wet which makes it not stay where you want it then again you end up with pink under wear. I also think it’s hard to close . I dropped it twice trying to close it. I don’t think they needed to try to make it fancy with having a push down to open cap. But so far I think it’s helping but I probably won’t order it again unless it’s on sale.

This lidocaine cream really does help to numb the pain. Not completely but enough to help without a doubt. I absolutely love that it’s made in the USA and with natural ingredients formulated by pharmacists. It’s very soothing.
Like everyone else, I only received 1/2 tub. There’s no way to measure if you’re getting the actual 2 oz or not. Item expires 09/2025
